Fees for Laser Vision Correction
Listed below are the fees for laser vision correction procedures. Please be aware that these fees are for a global period of one (1) year. If Dr. Lockhart cannot perform the procedure due to medical reasons, there will be no charge for the special testing. If you cancel your surgery after the special testing is completed, you will be charged for the special testing and exam only.
Fees are as follows:
- $2,700 per eye
- $5,400 both eyes
The above fees include the following:
- Initial eye examination
- Eye measurements
- Dr. Lockhart's fee
- Gailey Eye Clinic fee
- WaveLight Eye-Q measurements pre- and post-operative
- WaveLight Eye-Q Excimer Laser Platform fee
- Pre-operative prescription eye medications
- Post-operative prescription eye medications and supplies related to the procedure. Lubricating drops are over-the-counter eye drops and are not included.
- Post-operative examinations for one year (related to the procedure)
- One enhancement per eye, if necessary, within one (1) year of initial surgery
If you need to check with your insurance company regarding coverage, please indicate that the CPT code for LASIK/ASA is 65760. Although complications are rare, the patient's health insurance often covers them.
Payment by check (personal or cashier), Visa, Master Card, Discover or cash is due on special testing date or prior
One check to: Gailey Eye Clinic $400.00 per eye or $600.00 for both eyes
One check to: Lockhart LVC $2300.00 per eye or $4800.00 for both eyes
IRS ISSUES CLARIFICATION OF REFRACTIVE SURGERY TAX ISSUE: Revenue Rulings 2003-57. Uninsured costs for refractive surgery are a deductible medical expense for personal income tax purposes.
